Poison and paralysis will overwrite each other if inflicted on a player.

Poison will gradually drain a player's HP until it reaches 0 or until cured. The damage dealt by the effect scales with difficulty.

Poison does not wear off on its own. It will remain on a player even if they return to Pioneer 2, however it will not damage players who are on Pioneer 2. It can be cured with Anti level 1 or greater, with an Antidote, or with a Sol Atomizer. An equipped Rare will prevent players from becoming poisoned, but will not cure any ongoing poison when equipped. Android characters are immune to poison.

Monsters
Players have no means by which to directly apply the poison effect to monsters. A monster can become poisoned if it happens to be within the radius of a poison trap when it detonates, however this is a rare occurrence. Poison will function the same on the monster, gradually siphoning its health until it dies.

Traps are hovering hazards which explode when one or more targets come close enough. A trap will either deal fire-based damage to all targets within its area of effect, or inflict a status condition.

Traps are a feature of each zone, however they generally do not appear in Forest except in certain quests. They can be in rooms or hallways. Traps are often found in groups, and often guard loot boxes or switches. Traps are normally invisible until triggered, however a player can use a Trap Vision to reveal any traps within its radius. rare grants always-on trap vision while equipped. Android characters have permanent trap vision.

When a trap is triggered, it briefly becomes visible to all players, then explodes after a few seconds. A trap will either deal damage or will inflict one of poison, paralysis, confusion, frozen, or slow. Visible traps can be attacked with ranged weapons or techniques before they detonate. They are immediately destroyed when hit by any attack. Some traps may follow the target player around by hovering directly over their head before exploding; these traps must be targeted by a technique such as Zonde or attacked by a fellow player.

Traps used by players
All android classes can deploy traps of their own, namely damage traps, freeze traps, and confuse traps. Androids start at level 1 with 2 of each trap, and gain more of them as they level up, with each class gaining different amounts of each trap throughout level progression. All player-owned traps max out at 20 of each type of trap. They are replenished by leveling up, using a healing ring, the Medical Center, or when dying (whether you revive or respawn).

Player-deployed traps will affect monsters and not other players. After deploying they take a moment to "come online" and once they do so, they immediately detonate when one or monsters are nearby. The time spent to "come online" is reduced in higher difficulties. A player who deploys a trap can immediately detonate it by shooting it with a ranged weapon, such as a handgun or a needle.

Players cannot deploy traps in boss arenas.

The Medical Center is a facility on Pioneer 2. It is staffed by nurse NPCs in distinctive nurse uniforms. Players can visit the Medical Center and pay 10 meseta to be restored to health. This heals all HP and TP, restores any missing traps for androids, and cures all status conditions, including Shifta and Deband.

If a player dies on Ragol and is not revived in-place by any means, they will respawn at the Medical Center. On other versions of PSO, players would lose all meseta they were carrying when they respawned, however Ephinea does not impose any death costs on players.

On Episode 1 and Episode 4, the Medical Center is located near the teleporter to Ragol's surface, just to the right when returning from Ragol. On Episode 2, the Medical Center is near the shops.
