Anguish is a Diablo III-inspired game mode unique to Ephinea that allows players to challenge themselves with increased monster difficulty. There are ten Anguish levels, and any of them can be accessed by any character at Level 80 or above, just like Ultimate mode. Anguish is exclusive to Ultimate mode, and can be activated in the game creation menu at the lobby counter.

Freeze traps are not available to androids in any difficulty. Freezing specials function normally, albeit with the freeze time and activation rate reductions listed below.
Each Anguish level has a base percentage boost rate, ranging from 15% at level 1 to 300% at level 10. Most monster parameters are increased by the base rate shown in-game, but others are higher or lower depending on the stat:
